If you send an inquiry, we collect only what you enter: your name, preferred contact method, phone and/or email, state, county or ZIP code, and your general reason for contacting us. We use it only to respond to you.

What not to send

Please do not send medical information, Social Security numbers, insurance IDs or documents through this website or by email. This website is not a patient portal.

Who receives it

Your inquiry goes to the Care4U agency for the state you select. Submitting the form sends your inquiry to the agency's email inbox (contact@care4uusa.com) through our website form service, FormSubmit, which processes what you enter to deliver it and can retain a copy for up to 30 days. We do not sell your information.

Cookies and analytics

This website does not use advertising cookies. If we add website analytics, they will not include the content of inquiries or your personal details, and we will update this notice.
